editorial | Dr George Freedman Editor-in-chief 3D printing: Unveiling the future of dentistry The digital transformation of dentistry is driving the pro- fession faster than any previous technological revolution. It is transforming and reconfiguring the clinical practice of dentistry before our eyes. A change that was expected to take decades is now projected to unfold in a matter of years, or mere months. The innovations are startling. They are game-changing. They affect everyone. These massive dental paradigm shifts can be considered fright- ening or joyously astounding. The practitioner, educator, manufacturer or distributor who has the latter response will embrace these innovations and adapt them for the advancement of patient treatment and oral health. Digital dentistry is truly a worldwide phenomenon. The research, discoveries, adaptations and applications seem to spring up anywhere and everywhere that bright minds focus on current problems and future solutions. The key is to bring these visionaries, producers and consumers together so that they can catalyse each other to improve the developmental process, facilitate the path of manufacturing and deliver the finished prod- uct to the practitioner. Dental Tribune International’s 3D printing magazine has been at the forefront of shaping the written medium for communication and networking in this novel technology environment, both online and in hard copy, since 2021. Pertinent articles, highlighting the leaders in 3D development and their innovative clinical applications of the newest products, have made 3D printing a must-read for every forward-looking dentist. The Digital Dentistry Show, the biennial international platform for all things digital in dentistry (www.dds.berlin), will be held at the Arena Berlin events space in Germany on 28 and 29 June. With over 30 speakers, more than 120 exhibitors and well over 2,000 registrants, the Digital Dentistry Show will assemble the best and the brightest in the field. The Digital Dentistry Show promises to provide a unique setting enabling communication and collaborative interaction by the designers of software platforms, technologies and materials in a research and develop- ment space where the terminology is standardised and mutually comprehensible. The Digital Dentistry Show will serve as an incubator through which researchers, developers and manu- facturers can begin to establish a broadly recognised and internationally accepted framework of measurements, standards and guidelines canvassing the existing leadership of 3D printing at various levels. The most important goal is to formulate a basis for industry-wide compatibility, one that provides adequate headroom for flexibility and growth that can reasonably accommodate foreseeable (and perhaps unforeseen) upcoming developments and will build the bridge of continuity between the present and the future. news | and assistance from an orthodontist may not be required during each appointment. In comparison, an orthodontist may need to make appliance adjustments, for example to the archwires, every three to four weeks. to finish treatment because the aligner treatments could not adequately achieve certain tooth movements or correct common malocclusions.5 Aligners offer greater treatment comfort. Fixed appliances can irritate the oral tissue, whereas aligners only cause some discomfort when new trays are worn owing to the extra pressure applied. Patients treated with fixed appliances report greater discomfort and consume more analgesics than patients treated with aligners do.2 Fear of pain is one of the primary reasons that patients fail to seek orthodontic care, and pain has a negative effect on patient compliance. The business model adaptation of D2C Patients may be discouraged by the high cost of aligners. By cutting out in-person dental supervision and monitoring, D2C companies offer treatment for considerably less than the cost of an in-office procedure. Proponents of D2C orthodontics argue that the low cost of D2C aligners makes tooth alignment accessible to a greater number of individuals. However, D2C companies have been criticised for their inability to adequately treat malocclusion. In 2018, the American Dental Association passed a resolution against D2C tooth alignment,3 and the American Association of Orthodontists has publicly cautioned against the practice via a consumer alert and has filed legal complaints with 36 dental boards against SmileDirectClub, the leading D2C aligner company.4 In order to regain trust and improve their reputation, D2C companies have changed their business model. Whereas the D2C business model previously involved no visits to a dentist or an ortho- dontist, it is now common for D2C aligner patients to consult either an orthodontist or a general dentist who is employed by the company for an intra-oral scan. This adaptation to the business model results in higher costs for the patient. Customers will still receive their aligners through the mail. SmileDirectClub, for example, is introducing the CarePlus aligner offering this year, which offers customers the possi- bility of consultations with their treating dentist in-person or via the company’s teledentistry platform. This programme targets higher-income customers and offers them a hybrid approach including remote and in-person care. The CarePlus offering was priced at US$3,900 (€3,600) when the pilot programme was launched in the first quarter of this year, and further expansion is planned throughout the year. Expansion of the aligner market to general dentists Some orthodontists are sceptical about the efficacy of aligners in treating malocclusion. In some cases, for greater efficacy, patients need to switch from aligners to fixed appliances during the course of treatment. For example, one in six patients switched from aligners to fixed appliances Aligner companies are therefore expanding their products to general dentists for treatment of mild malocclusions or those requiring less complicated movement. Align Technology continues to increase its presence globally by making its Invisalign products available to more customers in more countries. Among other growth strategies, the company is seeking to enable general dentists, who have access to a larger patient base than orthodontists do, to easily identify potential cases they can treat with the Invisalign system and monitor patient progress while providing high-quality restorative, orthodontic and preventive oral care. Align is accomplishing this strategy by providing training and clinical education to general dentists and by offering digital tools such as iTero scanners. Conclusion The aligner market is forecast to grow at a double-digit rate in the coming years, and that growth is expected to be driven by the popularity and practicality of aligners, along with various efforts by companies to expand their market share. Although D2C companies have been criticised for addressing malocclusion inadequately, they are adapting their business model to involve consultations with dental professionals. Considering the sceptical stance taken by some orthodontists towards aligners, treatment providers are planning to expand their businesses to general dentists who have larger patient bases than orthodontists do. news | tained their structural integrity and strength; however, when subjected to low frequencies, resembling natural masticatory forces (1–11 Hz), the materials were more flexible and less likely to break, but also exhibited a decrease in strength. At this frequency, there was significant variability, suggesting these results might be unreliable. These findings point towards a potential reduction in the durability and reliability of these materials when subjected to the varying forces within the oral cavity. The biological assessment found a significant reduction in cell viability after 3 and 24 hours of exposure to these materials, indicating a need for cautious evalu- ation of these materials’ long-term interactions with oral tissue. This reduction in cell viability, however, showed some signs of recovery after 24 hours, suggesting a potential decrease in cytotoxicity over time. The research team suggested further exploration of the behaviour of these materials in artificial saliva to better simulate oral conditions and investigation into their use for temporary tooth- or mucosa-supported prostheses, including their resistance to microbial attacks and biofilm accumulation, particularly by Candida species. Further microscopic analyses were also suggested in order to identify any microfractures or structural changes after mechanical testing. “The study measured the viscoelastic properties of the materials by subjecting them to a dynamic load and analysing their deformation [...].” The understanding of these materials’ varying be- haviour under dynamic loading conditions and their interactions with biological tissue provides a foun dation for further research and development in the field of materials science, ultimately aiming to enhance the performance and safety of dental prostheses fabri- cated using additive manufacturing technologies. Once the design has been completed, a medical-grade resin is used to 3D-print each mouthpiece (Fig. 5). Clinical performance Finally, after post-processing and quality control, the completed mouthpieces are paired with the hydro station. These products are then shipped directly to the patient, along with a sample of Proclaim’s con- centrated oral infusion, to promote a feeling of freshness and thus compliance. In total, delivery typically takes a few weeks. The Proclaim Custom-Jet Oral Health System has been evaluated in a range of forums, including practice-based assessments and randomised clinical trials. The results of a controlled clinical trial with positive and negative controls were particularly impressive. After this step, scan bodies (TruAbutment) were added ready for 3D scanning at MUA–implant level using an Aadva scanner (GC; Fig. 3). A new long-term prototype was designed in exocad to the new MUA–implant level (Fig. 4). The screw access holes were corrected at this stage. This particular case was designed as one solid merged unit, which was then imported into the iBar module (Blenderfordental). Next, a one-piece merged de- sign from exocad was separated into a bar and shell design (Figs. 5–7) using the iBar module. Because of the long span of the edentulous area in the maxillary anterior, the bar was milled (DWX-52D, Roland DGA) out of TriLor techno-polymer (Bioloren), consisting of thermosetting resin reinforced with fiberglass oriented multidirectionally (Fig. 8). The case was prepared for 3D printing on a MAX UV printer (Asiga; Fig. 9) via the Composer 2.0 software (Asiga). There were a total of three short builds. The shell (Fig. 10) was 3D-printed using Rodin Sculpture nano- hybrid (Pac-Dent; Shade B1; Fig. 11). This 3D-printing resin has been de- veloped for fixed restorations and long-term provisional restorations for both permanent and temporary use. The maxillary implant model (Fig. 12) was 3D-printed using Asiga DentaMODEL (Fig. 13). user report | 27 28 29 Shaping is best done once the bar and the shell have been cemented together. The titanium bases and bar insert holes were lightly sandblasted at 300 kPa using 50 μm aluminium oxide. All parts were then thoroughly steam-cleaned and com- pletely air-dried. Finally, all parts were placed in an ultra- sonic cleaner in 99% isopropyl alcohol and then air-dried. G-Multi PRIMER (GC) was then applied and allowed to air-dry thoroughly (Fig. 20). It is crucial to ensure that all parts are completely dry before cementing. For cementing the titanium bases into the TriLor bar, G-CEM LinkForce (GC; Shade Opaque) was used (Fig. 21). Before starting the polymerising process, the ce- mented titanium bases and the TriLor bar were placed on the implant model, and MUA screws were inserted and hand torqued (Fig. 22). The model with the TriLor bar, titanium bases and screws were then placed in a light polymerisation unit that has dual heat and light polymerisation. The heat allows the dual-polymerising G-CEM LinkForce cement to activate the chemical polymerisation process, as light cannot penetrate the TriLor bar. In preparation for the cementing of the bar and shell, both were lightly sandblasted at 300 kPa using 50 μm aluminium oxide. Both parts were thoroughly cleaned with steam and completely air-dried. Finally, the bar and shell were placed in an ultrasonic cleaner and submerged in 99% isopropyl alcohol. After washing, the parts were air-dried. The screw access holes on the TriLor base were blocked with wax, and G-Multi PRIMER was ap- plied to both the shell’s intaglio surface and the bar (Figs. 23 & 24). Excess cement was removed using brushes soaked in 99% isopropyl alcohol (Fig. 25). glazing and characterisation, OPTIGLAZE color (GC) was used (Fig. 26). The arch was then polymerised in an Otoflash G171 (NK Optik) using 500 flashes in an inert atmosphere, created using nitrogen. A final glaze was then applied using OPTIGLAZE color (Shades Clear and Clear HV), sealing and protecting the characterisation (Figs. 27–29). It is vital to use OPTIGLAZE in Shade Clear HV on all intaglio surfaces that will be touching the oral tissue. This seal protects the tissue from the TriLor, and at no time should unsealed TriLor be exposed to the oral environment. In light of the patient’s request for a fixed denture, the options were a bridge from tooth #13 to tooth #16 or implants in regions #14 and 15 with subsequent crown restoration of the implants and tooth #16. Based on the integrity of tooth #13, the patient opted for an implant restoration. This was followed by comprehensive briefing on the clinical procedure and the taking of a CBCT scan and an impression of the situation. Treatment planning Preference should always be given to a CBCT scan with a small field of view (CS 9300, Carestream Dental; 5 × 5 × 5 cm, 78 kV, 6.3 mA, 20 seconds). This makes it possible to reduce the patient’s exposure to radiation and achieve a smaller voxel size, which equates to a higher level of detail. A cotton roll is inserted in the buccal region for better matching of the DICOM and STL data sets via the soft tissue in the CAD software. The STL data set is obtained by means of an intra-oral scanner or in-laboratory scanning of the plaster model. The prosthetic restorations were first designed in planning software (Implant Studio, 3Shape). The DICOM volume data set (from the CBCT scan) was then merged with the STL surface data set (from the intra-oral scan; Fig. 2), and the implants were aligned on the basis of the prosthetic restorations (Fig. 3). The vertical dimension in region #14 was 10.5 mm and decreased distally from 5 to 7 mm in region #15. Straumann Standard Plus implants were planned for region #14 (3.3 × 10.0 mm) and region #15 (4.3 × 8.0 mm). The use of implants of these lengths would require an internal sinus lift. In order to allow guided preparation of the osteotomy to just before the maxillary sinus and the Schneiderian membrane, implant #15 was moved coronally in the planning software and its length shortened. The planning was completed with the creation of the surgical guide and the corresponding drilling protocol. Production of the surgical guide Importing the STL surgical guide data set into the corre- sponding nesting software makes it possible to align the surgical guide and furnish it with supporting structures (Fig. 4). The slicing of the surgical guide is performed automatically based on the material to be printed and the printer. In this case, we used the transparent 3D-printing material V-Print SG (VOCO; Fig. 5) in combination with the D20 II DLP printer (Rapid Shape). Printing is followed by post-processing, involving ultrasonic cleaning in iso- propanol and light polymerising, to achieve the final material characteristics of the surgical guide (Fig. 6). The provisional bridge was modified at the base to create space in case of swelling and inserted with metha crylate- based temporary luting material (Bifix Temp, VOCO). The screw-retained final restorations were fabricated from a multilayered monolithic zirconia (DD cubeX2 ML, Dental Direkt; Fig. 14). Discussion Placement of an implant in a suboptimal position can have effects on the osseointegration, cleanability and function of the implant. In addition to aesthetic compro- mises in the prosthetic restoration, an inadequate implant position may be associated with functional issues and an increased risk of peri-implantitis.7, 8 In order to achieve a prosthetically and biologically adequate implant position, surgical guides are used nowadays to transfer digital planning to reality. The ma- terials used for the printing of surgical guides are usually methacrylate-based and differ in their properties, such as the modulus of elasticity. The precision of guided implant surgery is usually defined as the discrepancy between the planned and actual postoperative clinical position of the implant. Equally good results in trans- fer precision have been obtained in studies with milled and printed guides in edentulous spaces such as in the presented case.9, 10 Sterilisation at 135 °C for 5 minutes had no significant effect on the material used.9 However, the surgical guide material and printer used did have a significant effect.9 In in vivo studies, deviations have been evaluated with implants placed with surgical guides and been found to be significantly below the deviations using freehand procedures.11 In addition to positioning, surgical guides facilitate the procedure for the surgeon, as demonstrated in this case. Corresponding planning allows guiding of the drill up to just before the maxillary sinus, allowing more efficient fracturing of the cortical bone of the maxillary sinus floor with an osteotome and lifting of the Schneiderian membrane. This shortens the overall duration of the surgery, making it more acceptable and pleasant for the patient. and more! However, the beauty of 3D printing is that applications are only limited by your imagination, restrictions in design software and the materials currently available on the market. It gives the clinician and technician the opportunity to let their creativity run rampant, all while improving the patient experience. In this case report, I will share how CAD software and CAM hardware can create a predictable and innovative provisional implant restoration which satisfies not only the patient but also the clinician and dental designer– technician. Initial situation A male patient presented in our dental office with a severely broken maxillary right first premolar (tooth #14) that had been designated for extraction a number of years before. The tooth immediately distal (tooth #15) was also in need of substantial dental treatment (Figs. 1 & 2). The patient had hypertension that was controlled with anti-hypertensive medication, but was otherwise med- ically fit. He intended to have the colour of his anterior teeth improved, but wished to address the issues on his maxillary right side first. Tooth #14 was deemed unrestorable, and so it was decided to extract the tooth (Figs. 3 & 4), wait for bone healing and replace it with an implant with a provisional restoration. During the healing process, endodontic therapy would be performed on tooth #15. After bone integration, implant #14 would be restored with a screw-retained direct-to-implant zirconia crown, and tooth #15 would also be restored with a complete zirconia crown. A deliberately lighter shade would be chosen, factoring in the patient’s long-term desire to improve the colour of his teeth. Implant planning Two months after tooth #14 had been extracted, a CBCT scan (GO, NewTom) and intra-oral scan (Medit i700 wireless) were performed, and the two data sets were merged on 3Shape Implant Studio in order to plan the patient’s case (Fig. 5). The use of surgical guides has been shown to enhance accuracy and precision for many users.1 A tissue-level matrix implant (4.1 × 8.0 mm; TRI Dental Implants) was digitally positioned in order to design the surgical guide, based on parameters already pre- loaded on the extensive 3Shape Implant Studio library. A pilot guide was designed in this case, as it has been shown to produce similarly accurate results in compar- ison with fully guided systems.2 The surgical guide was printed out of V-Print SG (VOCO) on the MAX UV (Asiga) in 50 μm layers and was autoclaved after complete processing. case report | 7 6 8 Fig. 6: Provisional restoration design on 3Shape Implant Studio. Figs. 25a–f: Situation at each step of treatment. provisional restoration revealed a natural soft-tissue profile and an improved soft-tissue contour without the need for additional soft- or hard-tissue augmentation (Fig. 21). Secondary impression was taken digitally with an intra-oral scanner and the relevant scan body. Definitive restorations were designed on exocad and milled in-house on CRAFT 5X (DOF) out of EVEREST Multilayer AT zirconia (Shade A3; UNC International). A deliberately lighter shade was chosen by the patient, as he intended to have the appearance of his other maxillary teeth improved. The implant crown was torqued in to 35 Ncm, and the crown was cemented to tooth #15 using G-CEM ONE cement (GC Dental; Figs. 22–24). Conclusion Digital dentistry and novel techniques can help pro- duce excellent and effective results. Improvements in the software and physical workflow have meant that the clinician’s and the technician’s work are not only more streamlined and straightforward but also enjoyable (Figs. 25a–f). A more-than-satisfactory outcome was achieved for the patient. What made all this possible was the novel and innovative tissue-level matrix implant connection, which lacks any sharp corners or edges, allowing for direct-to-implant milled and printed restorations. Overall, 3D printing and software can unlock numerous applica- tions to improve not only the patient outcomes but also the operator experience. It will be truly exciting to see the innovations that lie ahead. The greatest oppor- tunity for facilitating the transition to digital dentistry is through providing education and information that seek to reduce the time and effort required to learn to use new technologies. Here, education such as offered by the Digital Dentistry Society has a role, especially be- cause it is currently the largest scientific society in digital dentistry.
