“Retreatments are a unique way to save not only teeth but also the surrounding bone” An interview with Dr Ahmed Shawky el-Sheshtawy, AEEDC Dubai 2024 speaker. interview journey beginning with the rationale be- hind retreatments, progressing to ensur- ing rational fulfilment and finally the de- cision-making process, which is the most critical process in our treatments. My lec- ture will also cover different procedures and techniques used to manage complex retreatment cases. Could you explain how the objectives of retreatments, like the healing of periapical periodontitis and its preven- tion, are more complex compared with primary treatments? The treatment and prevention of recurrent periapical periodontitis in failed treatments are notably more com- plex than in primary treatments. In pri- mary treatments, the root canal system, despite its intricacies, can be effectively managed with current shaping and cleaning protocols, achieving success rates as high as 95% in most instances. However, in cases where treatment fail- ures necessitate retreatments, additional factors or challenges may complicate the situation. These include the removal of previous root filling materials, altered anatomy, the presence of perforations, canal blockage (iatrogenic or biological), inflammatory resorption and the in- ability to achieve patency across the foramen, which is crucial. Furthermore, the presence of an apical lesion adds to these complications. These complications, combined with the inherently complex nature of the root canal system, make retreatments significantly more challeng- ing than primary treatments, often re- sulting in lower success rates. What are the key challenges clinicians face in gaining access to the root canal system during retreatments, espe- cially considering factors like existing intra-canal fillings and altered canal anatomy? 1a 1b 1c  Figs.1a–c: (a) Mandibular left second molar presented with unsuccessful primary treatment and symptomatic periapical periodontitis. (b) Non-surgical treatment involved an apical plug in the distal root. (c) One-year follow-up showed complete healing of the periradicular lesion. Regaining access to the root canal system can be challenging, especially when fibre posts have been used as intra-canal retention elements. Clini- cians need the requisite experience and appropriate tools to avoid iatrogenic er- rors. Regarding intra-canal filling mate- rials, a different yet dynamic approach is required. This is because there are various types of root filling materials, including the older silver points, the widely used gutta-percha and calcium silicate materials, each requiring a dis- tinct method of removal to regain full access to the root canal system. In- adequate removal of these materials during retreatments can compromise the treatment outcome. Remnants may limit the accessibility of disinfectants and instruments to micro-anatomies or may result in an inadequate seal. Given the additional challenges, what are the key factors to consider when developing a strategy for a successful retreatment outcome? Key factors for developing an effective retreatment strategy include a thorough pretreatment evaluation by the clinician. This evaluation is crucial for determining the case prognosis and formulating the treatment plan. The next step involves decision-making, where the clinician decides on the retreatment approach—whether non-surgical, sur- gical or a combination of both. In this context, the clinician also has to decide whether the tooth presented can be saved. The third factor encompasses the technicalities and procedural steps involved in the retreatment process. What advancements in endodontic tech- nology or techniques have improved the outcomes of retreatment proce- dures in recent years? Endodontics is a very dynamic and rapidly evolving specialty. It has bene- fited from advancements in imaging modalities, such as CBCT, which ena- bles a precise visualisation of dental issues. Recent advances in nickel– titanium instruments and their kine- matics, along with the introduction of new irrigant delivery and activation instruments and devices, have contributed to improved treatment outcomes. The integration of magnification and ultrasound in endodontic practice is a critical aspect that has significantly transformed the specialty—I consider these to be game-changers. These technologies have been instrumental in various procedures, ranging from disassembly to cleaning, and even in correcting previous mishaps. In addition, I would like to highlight the rapidly evolving advancements in endodontic biomaterials, particularly calcium silicate materials. These have proved helpful in complex retreatments, especially in cases involving root defects or challenging anatomies. Additionally, there have been significant advance- ments in concepts and materials for the post-endodontic restorative phase, and these play a crucial role in extending the survival time of retreated teeth. How do you manage patient expectations and communicate the potential risks and benefits of undergoing a retreatment procedure? That’s a very good question, espe- cially for young clinicians. The key fac- tor in managing patient expectations lies in proper pretreatment evaluation. This involves examining the current status of the tooth, assessing potential complications and, most importantly, evaluating the survival potential of the tooth after treatment. Equipped with this information, clinicians can clearly and easily communicate the benefits and risks involved to patients. Is there anything else you would like our readers to know? I would like to emphasise that retreatments are a unique way to save not only teeth but also the surrounding bone. VR and AI prove useful aids in treating patients with mental disabilities Study highlights impact of virtual reality and artifi cial intelligence on dental care. news  Every dental clinician in the world will, at some point, treat a patient with a form of mental dis- ability, and according to recent re- search, technology is now available to make that treatment go more smoothly and to help the patient feel more comfortable. This study, signifi cant in the context of Saudi Arabia where over a million indi- viduals suff er from mental dis- abilities, aimed to address these patients’ inability to express dental treatment needs. The researchers explored the impact of virtual real- ity (VR) and artifi cial intelligence (AI) interventions during dental treatments on anxiety and behav- ioural responses in patients with mental disabilities. The study begins by highlight- ing the high prevalence of mental disorders globally and the specifi c challenges faced by individuals with mental disabilities in accessing dental care. The technological ad- vancements in mental health treat- ment, especially through AI and VR, are underscored as potential solu- tions for managing anxiety and improving behaviour in dental set- tings. The authors sought to determine whether immersive VR experiences combined with AI interventions could reduce anxiety and improve behaviour in individuals with mod- erate to severe mental disabilities, such as cerebral palsy and Down’s syndrome. It hypothesised that VR simulations would familiarise individuals with the dental environ- for each patient as well as the effi cacy of the programme during each phase of the treatment—pre-intervention, intervention and post-intervention. By additionally using the Frankl Behavior Rating Scale and the Venham Anxiety and Behavior Scale to assess behavioural changes, the clinicians were able to note a signifi cant decrease in anxiety lev- els and improvement in behaviour when VR and AI interventions were employed. For example, GSR scores reduced signifi cantly from pre- treatment to post-treatment. Addition- ally, there was a notable improve- ment in behaviour shown in the Frankl and Venham scores, and the study found a positive correlation between reduced anxiety and im- proved behaviour. According to these results, it seems that VR-based distraction is an eff ective method for managing anxiety and behaviour during non-invasive dental treatments in individuals with mental disabilities. It suggests that VR and AI interven- tions have broader applicability in various clinical settings and calls for further research to explore their eff ectiveness in more complex medical and dental procedures. The study, titled “Eff ect of virtual reality and artifi cial intelligence on anxiety and behaviour among indi- viduals with mental disabilities in a dental setting”, was published on 27 October 2023 in the Journal of Disability Research.  ment, thereby making minor pro- cedures easier for dental staff and patients. The cross-sectional study was conducted between December 2022 and March 2023 among 90 Saudi female residents in a rehabilitation centre. Participants were selected based on specifi c inclusion and exclusion criteria, and data was collected on their demographic background, baseline anxiety lev- els, and behaviours. Two forms of intervention were employed in the care of these patients. The fi rst uti- lised an Oculus Quest 2 VR headset to create a simulated natural envi- ronment to provide a soothing ex- perience. The second intervention involved an AI-powered environment linked to a galvanic skin response (GSR) sensor, which adjusted the environment based on the patient’s anxiety levels by measuring emo- tional arousal. The simulated environment cre- ated utilised the relaxing properties of nature such as imagery of a river and wetlands. Additionally, cartoon characters were included, and the environment was part of a custom- ised design. The GSR monitored whether the patient reached an established threshold and adjusted the VR environment to compensate and provide a more soothing ex- perience. Each anxiety score was tailored to the patient, and the authors did not assign a baseline score. These individual scores were useful for evaluating before and after status Wanted: Research on 3D-printed zirconia restorations Researchers conduct analysis of 3D-printed vs. milled zirconia prostheses.  Zirconia-based restorations, re- nowned for their durability and aes- thetics resembling natural teeth, have gained popularity over the past 20 years and are increasingly being produced using CAD/CAM. A recent systematic review and meta-analysis comparing milled versus 3D-printed zirconia prostheses has found continual gaps in the research, including biases, and potential under– representation of the performance capabilities of 3D-printed restorations. A complete lack of clinical studies on the effi cacy of 3D–printed zirconia crowns and fi xed dental prostheses (FDPs) also high- lights how far research into zirconia crown fabrication has yet to go. Zirconia, dubbed “ceramic steel” for its robustness, is a preferred choice for various dental restorations owing to its high strength and biocompatibil- ity, among other attributes. The intro- duction of CAD/CAM technologies has enhanced the precision of manufac- ture of zirconia restorations by mini- mising human error. There are two main fabrication methods: subtractive (milling) and additive (3D printing). Each technique has its advantages. Milling is cost-eff ective and less sen- Clinically, zirconia crowns and FDPs showed impressive biological and physical properties, with few reported issues. The meta-analysis revealed that, regardless of varia- bles like age, glazing/staining, lo- cation and marginal integrity, the pooled survival rate of these crowns and FDPs was consistently high, at 100%. In the context of in vitro studies comparing 3D-printed and milled zirconia crowns and FDPs, fi ndings suggested that both meth- ods off ered comparable accuracy, adaptability and strength, positioning 3D printing as a viable option for clinical use. The main biological outcome observed across the board was bleed- ing on probing. Technical issues, such as crown fractures, were noted, one study recording a survival rate of 91.5%. Proper polishing was found to reduce the chance of crown fracture. Other key fi ndings in the review were that patients with brux- ism should be treated with zirconia crowns and FDPs with great caution and adjustments might be needed after crown placement owing to oc- clusal issues or pain. The average preservation of marginal integrity was high, but varied depending on the study. Zirconia was found to cause wear on opposing teeth, but less so than other ceramics. There is limited literature on zirconia’s aesthetic aspects, especially concerning colour. The review faced challenges like technique variability and limited patient sample size. Con- clusively, zirconia crowns and FDPs, whether 3D-printed or milled, are promising but require more exten- sive research, especially long-term studies, to validate their benefi ts over traditionally produced prostheses. Although in some cases the literature suggests that milled crowns and FDPs are superior to 3D-printed ones, there were no identifi ed direct com- parisons performed in any studies. The lack of research into 3D-printed crowns and FDPs may have contributed to this result. The study, titled “Clinical eff ec- tiveness of 3D-milled and 3D-printed zirconia prosthesis—a systematic review and meta-analysis”, was published on 27 August 2023 in Biomimetics.  sitive to minor design imperfections, while printing off ers more design fl ex- ibility and effi ciency in intricate struc- tures. While 3D printing of dental res- torations is nascent, studies compar- ing the eff ectiveness of milled versus printed zirconia are scarce. The present review was conducted in an eff ort to bridge this knowledge gap by seeking to determine whether the performance of zirconia crowns and FDPs produced by both methods com- pares favourably with that of conven- tionally produced ones. The review in- cluded various in vitro studies, neces- sary in lieu of any available clinical stud- ies on 3D-printed zirconia restorations, and the results are intended to guide cli- nicians in selecting the best method for creating crowns and FDPs. The included articles were published from 2004 to November 2022 and reported on the placement of over 1,500 zirconia resto- rations, predominantly in the posterior regions. Middle Eastern dental market to swell to more than US$1.6 billion By Donna Santos & Dr Kamran Zamanian, iData Research news  Although the dental industry in the Middle East plunged in value by more than 30% in 2020 owing to the COVID-19 pandemic, by 2023 it had rebounded so signifi cantly that predictions hold the market to be worth upwards of US$1.6 billion by 2029.1–5 The industry includes a variety of market segments, such as dental prostheses, digital den- tistry, dental bone graft substitutes and other biomaterials, dental implants and dental operatory equipment, and continuous developments have turned what was a sinking market into some- thing robust, promising valuable gains for the region, which includes Bahrain, Iran, Israel, Kuwait, Oman, Qatar, Saudi Arabia and the UAE. Dental prostheses market The dental prostheses market, encompassing crowns and bridges, inlays, onlays and complete dentures, is the largest segment. It is expected that the demand for dental prostheses will continue to grow, thanks to an expanding regional population, which has increased by more than 35% since 2005.6 It is the aged among the popula- tion which has signifi cantly bolstered this market’s growth. According to data published by the World Bank, it can be estimated that the UAE’s ageing population has been growing at an average annual growth rate of 19%, followed by Kuwait at 12%.7 Moreover, this demand is fuelled by the growing number of migrant workers, as Arab states are considered a top destination in oral health, may have a negative impact on market growth. A Pew Research Center article reported that 93.9% of residents in the UAE, 80.6% of resi- dents in Qatar and 71.3% of residents in Kuwait in 2020 were international migrants.8 Overall, the dental prosthe- ses market is expected to reach over US$900 million in 2029, the majority being accounted for by crowns and bridges.2 Dental implant market In line with the growth of the dental prostheses market, the dental implant market is poised to reach US$187 million.5 Improved access to dental healthcare is expected to con- tinue boosting market demand in the short term, allowing patients suff ering from edentulism to receive implants with ease and reliability. Based on estimates of per capita expenditure on dental healthcare, Israel has the highest per capita expenditure compared with other countries in the Middle East, followed by Qatar. The root canal system is an open path- way between the oral bacterial fl ora and multiple sensitive tissues and organs in our body. It is a dynamic science that we learn about it every day and poses new challenging cases and enigmas to solve. Every material and technique used should consider the biocompatibility and longevity of the treatment and of course how to preserve the tooth structure whether internally or externally. Simple yet very effi cient techniques should be part of our daily practice. Starting with the diagnostic part, 3D imaging is an essential tool to complete the puzzle and fi nd answers and to prepare a full treatment plan for a simple case as well as a compli- cated one. Artifi cial intelligence (AI) is a great addition regarding 3D imaging. For some time, I have been using the 7  Fig. 7: The ZenFlex and Traverse fi les (Kerr Endodontics). DTX Studio Clinic software, which has many AI features and enhanced im- age features that help me a great deal in my daily practice. When it is com- bined with the correct resolution and fi eld of view for endodontics, it be- comes a very powerful diagnostic tool. Digital 3D imaging allows the discovery of unusual anatomy. In the case of a patient who presented complaining of pain and pressure on his tooth after undergoing root canal therapy some time before, careful reading of a CBCT scan of the area revealed a second canal in the central incisor and enabled us to locate the furcation (Figs. 1–3). Cryotherapy in endodontics, an idea that I began applying several years ago, has snowballed. It involves using cold water in order to minimise postoperative endodontic discomfort to try to replace or minimise the use of analgesics. This is a very simple yet very eff ective solution to help our patients to overcome pain after under- going root canal treatment. Irrigation in endodontics is widely misunderstood, yet it is so simple when we understand the chemicals that we are using and what our goal is and how to achieve it. By giving each chemical the work or the target that it is supposed to achieve and by understanding chemical interactions— irrigation is a matter of dealing with diff erent kinds of chemicals—we can achieve better, much faster and safer root canal disinfection. Before the use of sodium hypochlorite, it took up to 20–30 minutes, and now it takes sec- onds. A bacterial level of zero is in our reach, and it does not take rocket science to achieve it. Zero bacteria means that we can have a better prog- nosis for the root canal treatment and higher success rates for our retreat- ment as well in a single visit and no temporary dressing needed—the myth of calcium hydroxide is no more. By way of demonstration, consider the case of a necrotic molar with a relatively large radiolucency that we treated with a root canal treatment in a single visit following our irrigation protocol (Figs. 4–6). clinical AEEDC Programme highlight The AEEDC Dubai Endodontic Forum takes place on 6 February from 9:00 to 17:00 at the Al Multaqa Hall. Nickel–titanium (NiTi) rotary fi les were a great invention that changed the world of endodontics, and the science of this alloy keeps on devel oping. In 2006, the fi rst heat-treated fi les were released, a game-changer in the production of rotary fi les. The changes brought addi- tional fl exibility and torque resistance, increasing the safety of using NiTi rotary fi les. More recently, through yet another inno vation in this fi eld, fl exibility and cutting resistance were combined in one fi le. The ZenFlex and Traverse fi les (Kerr Endodontics) have opened the way for a new era in fi le pro duction. This innovation will give us the oppor- tunity to use fi les in a very simple se- quence whether in primary treatment or retreatment (Fig. 7). Zero apicectomy, the art of com bining root canal treatment and microsurgery to preserve tooth integrity and no longer removing the apex, has been proved a very successful technique not only in preserving the tooth structure but also in recreating a new periodontal ligament where the bone can grow vertically—an idea that was impossible before—in order to save the natural teeth, a new approach in endodontic microsurgery. In a trauma case in which there was an absence of cortical bone and apical bone around the mandibular central incisors and a very deep pocket reaching the apical area, a zero apicectomy and a root canal treatment were done, and 18 months later, against all odds, the bone had grown verti- cally, covering the buccal part of the root almost completely, and the apical bone was completely restored (Fig. 8). Oral Reconstruction Foundation— new scientific knowledge for the benefit of patients An interview with Dr Martin Schuler, executive director of the Oral Reconstruction Foundation. interview  Dr Martin Schuler, executive director of the Oral Reconstruction Foundation, is looking forward to inspiring discussions with AEEDC Dubai 2024 attendees. AD Easy application into the sulcus  Originally established in 2006 as the CAMLOG Foundation in Basel in Switzerland, the Oral Reconstruction Foundation (ORF) was relaunched under its current name in 2016, and its US section was initiated that same year. In this interview, exe- cutive director Dr Martin Schuler speaks about the society’s goals and future developments as well as its expectations for AEEDC Dubai this year. Dr Schuler, could you please give us deeper insight into the ORF’s objectives? From training and access to re- search to the world’s most exclusive network of experts, the ORF opens the doors to implantology and oral tissue reconstruction. We support talented young scientists and prac- titioners as well as projects in the field of basic and applied research. We also promote scientific exchange between universities and industry, especially in the field of clinical re- search. We do this through education and training, lectures, seminars, practical courses, live surgeries and surgical partnerships. We are convinced that close international collaboration between universities, practitioners, technicians and the industry is the basis for the further development of the scientific and educational foundations of implan- tology and oral tissue reconstruction in order to achieve our goal of bring- ing together science and education to better serve the patient. How do you envision the future of the ORF in the upcoming years? Under the leadership of our current president, Dr Luca Cordaro, the board of the ORF has developed a vision for the next five years: we want to become an established platform for all dental professionals to elaborate and shape the latest de- velopments in implant dentistry and oral tissue reconstruction. To achieve this, we launched several new initia- tives last year, including our OR Forums, during a week-long roadshow in five cities in China. We are also in the process of establishing regional organisations, which we call “OR Districts”. We have made a start in China, the US, Germany and Spain. Regional organi- sations in Japan, the Middle East and Africa will follow soon. In all these districts, we will engage local key opinion leaders to organise educational events under the ORF umbrella. In addition, the ORF has sup- ported research grants since its be- ginnings. We have funded around 200 research grants, to the amount of over €5 million, which have led to over 230 publications. More than 60 research projects are currently underway, and we will keep investing into research. Which educational events are on the agenda this year? We have a full and varied pro- gramme of events this year—from round tables to symposia—but I am most looking forward to our first two international symposia in the Asia Pacific and Middle East and Africa regions. Under the theme “The age of speed”, we will start with our sympo- sium in Abu Dhabi in the UAE on 3 and 4 May. The scientific committee, led by internationally prominent periodontist Dr Homa Zadeh and Dr Abdelsalam Elaskary, the current president of the Arabic Society of Oral Implantology, has put together a great programme with top-class speakers to attract dental professionals from around the globe who seek to stay up to date with the latest indus- try standards, strive to develop new methods and learn from the best. We will then round off the year with our second international symposium in Bangkok in Thailand from 14 to 16 November. What do you personally expect from your participation at AEEDC Dubai 2024? In order to meet the needs of our local partners, key opinion leaders and other dental professionals, an open conversation on a personal level is essential. The Middle East is a fast-developing region, and my top priority at AEEDC is therefore to meet with as many people on-site as possible. The next show will be held from 8 to 11 May at the Istanbul Expo Center this year. In 2024, IDEX Istanbul will expand the exhibition area to meet the increasing demand, taking place in six halls and a spacious area of 60,000 m2. Last year, a record number of 87,570 visitors, including 28,460 visitors from over 80 countries, attended the event, generating a business volume of over US$200 million. Bringing together national and inter- national leaders in the fi eld of oral health in the enchanting atmosphere of Istanbul, IDEX Istanbul 2024 will connect over 500 exhib- itors with dental professionals from all over the world, showcasing the products of more than 4,000 brands. The organisers expect to welcome over 80,000 visitors, including dentists, chief physicians and buyers, during the four-day event. Thousands of dentists and dental stu- dents will also have the opportunity to attend numerous training sessions conducted by ex- perts through the courses and symposiums held concurrently with the exhibition. Since its fi rst show, IDEX Istanbul has been a pioneer in its fi eld, boasting a participant and visitor profi le that is of a global standard. Through the business-to-business matchmaking sessions held at the exhibition, it has sup- ported global business agreements. The ten million-litre aquarium tank is home to thousands of animals of over 140 species, in- cluding more than 400 sharks and rays—there are sand tiger sharks and even giant groupers! Visitors may watch the aquarium divers feed the sharks and rays throughout the day. The best way to explore the huge tank is via the 48 m tunnel, which takes visitors 11 m below the sur- face. The underwater zoo is located above the aquarium tank and leads visitors through three ecological zones: rainforest, rocky shore and living ocean. This part is home to otters, piranha, lionfi sh, giant spider crabs, seahorses, garden eels, archerfi sh, water rats, paddlefi sh, cichlids and many more amazing aquatic animals.
