However older patients with dentures are not al- ways helped by extensive implant therapy. Problems with oral hygiene, particularly in the elderly, is a limit- ing factor. A removable prosthesis, adequate cleaning of a small number of implants in the anterior region and simple mouthwash is often easier to get to than with interdental brushes, floss and more extensive brushing. A well proven alternative is the Atlas system. The technique is based on implants, which support and hold the prosthesis in place. The place- ment of the minimally invasive im- plants usually do not need any major surgery as they require a minimum of space in the bone. A soft liner of re- silient silicone is used in the existing denture. It serves both as retention of the denture around the round im- plant head and as a shock-absorbing cushion against the gums. Further- more, no o-rings or rigid strongholds in the prosthesis are needed. The treatment is usually done during the same visit. Normally, the patient leaves the clinic with a firmly fittingdenturewithinanhour.During the next two days, the denture has to stay in place all around the clock. Patient should only drink liquid or very soft food. After these two days, the denture can be taken out for nor- mal cleaning, however, it should be put back immediately in order not to jeopardize the healing of the fixed implants. After a few weeks, the healing is well underway and the denture can betakenoutduringthenight.Thesoft silicone material sits with undercuts in the denture, and can easily be re- moved and replaced by the dentist when needed. Normally the soft re- linesiliconeholdsfor12to18months. Cleaning the denture does not differ much from ordinary dentures. A soft denture brush and a suitable toothpaste is all that is needed. Ob- viously, the implants also need to be kept clean. A soft toothbrush and floss will take care of that. The system has been studied at Sahlgrenska Institute in Gothen- burg1 , where the study shows an almost 100 % acceptance with im- proved speech, comfort and chewing characteristics with the patients in- cluded in the study. Improved self- confidence and self-esteem were other positive effects.
