I case report _ CT in treatment planning Anotherimportantpointtobeobservedisthepatho- physiologicalmechanismthatresultedinpulpnecrosis. Wesuspectedanapicalorperiapicalresorptionoftooth #21 because of the expansion of the erupting follicle andsecondaryosteolysis, whichcannotbeexcluded. In ordertoeliminatethegreatestnumberofcellsinvolved in the resorptive-destructive process, an apicectomy was performed. Nevertheless, pulp congestion sug- gested that the most probable pathophysiological mechanism involved was venous stasis of the vascular plexusthatenterstheincisor, justbeforeapex. Thelastpointofdiscussioniswhenthesesupernu- merary teeth should be removed. If possible, the best timeforremovalisbeforeanypathologysignsappear. This requires consideration of the individual case of eachpatient, andperformingclinicalandradiographic follow-up of the case in order to determine the right time. _Conclusion The presence of supernumerary teeth in the per- manent dentition has a frequency of between 0.1% and 3.8%. Necrosis of the adjacent teeth is one of the possible complications of this phenomenon; therefore, cliniciansmustconsiderthepossibilityofa supernumerary tooth during diagnosis, especially in patients with pulp necrosis without previous trau- matic dental pathology._ 30 I cone beam1_2015 Dr Javier Martínez Osorio graduated in medicine in 1981 from the University of Barcelona in Spain. He spe- cialized in dentistry in 1983 and in plastic surgery in 1987.
