Science & Practice Friday&Saturday, 30–31 August 2013 T he concept of minimum inter- vention in dentistry (MID) ranges from early diagnosis of oral disease to appropriate inter- vention, which includes prevention, control and treatment for the pur- poseofconservingnaturaltoothand periodontal structure. A number of MID measures have recently be- come available, including the very early detection of dental caries us- ing QLF (Quantitative Light-induced Fluorescence)inordertodetectmin- eral loss in enamel, as well as fol- low-up strategies, such as reminer- alisation and the use of sealant. Moreover, the visual FDI Caries Ma- trix, in terms of non-cavitated and cavitatedlesionsinenamelandden- tine, has been proposed as a caries index for timely prevention and treatment. This early detection and diagnosis of dental caries will lead to proper prevention and control of caries before the development of a cavity, which can then only be treated. For caries restoration, partial caries removal during cavity prepara- tion has been suggested, which, ac- cording to research, appears to re- duce the incidence of iatrogenic pulp exposureandthereforetheriskofpain andinfection.Simplifiedandmodified atraumatic restorative treatment, a further development of atraumatic restorativetreatment(oneoftheorig- inalMIDmethodsofrestoration)anda preventive method of restoration for primary teeth that entails partial caries removal and filling with encap- sulated self-curing glass ionomer ce- ment, has been proposed. This con- cept makes preventive restoration in preschool children, even by trained dental auxiliaries, possible. More- over,effectiveuseofself-carefluoride toothpaste during toothbrushing has been suggested for every age group, especiallythecorrectminimalamount of toothpaste used in children to pre- ventbothcariesandfluorosis. There are several interesting MID measures in terms of periodontal dis- ease, such as non-surgical or mini- mally invasive surgery in periodontal therapy,andsingle-flaporflaplessim- plant surgery. The single-flap ap- proachisaminimallyinvasivesurgical procedureforthereconstructionofin- traosseous periodontal defects with early wound healing and highly pre- dictablecompleteflapclosure.Theef- fectiveness of the buccal single-flap approach for surgical debridement of deep intraosseous defects has been shown to be comparable to the dou- ble-flap approach in terms of clinical attachmentlevelgain,probingpocket depth reduction and minimal gingival recession, six months post-surgery. Another minimally invasive surgery method is flapless implant surgery conducted with help of the tissue punch technique instead of having to raise a mucoperiosteal flap. Reports show that this reduced operational time, accelerated post-surgical heal- ing and even increased patient com- fortinsomecases. MID can be applied in many areas of clinical dentistry. Importantly, it should be part of a science- and evi- dence-based practice that is able to providedentalservicesthatareacces- sible to, acceptable to and affordable foranincreasingnumberofpeople.
