research I I 19implants1_2013 Fourminiimplantswithadiameterof2.1mmand alengthof13mmwereinsertedinthemandiblefol- lowing the procedure described above. Their posi- tion was checked on the OPG (Figs. 11 and 12). Af- terwards, SECURE Soft Reline Material (3M ESPE) was applied into the denture base, which was cau- tiously placed into the mouth of the patient. In this way, the implant positions were marked in the ma- terial (Fig. 13). Subsequently, the metal housings were embedded in the denture base using SECURE HardPick-UpMaterial(3MESPE).Thehousingswere pressed into the denture intraorally during occlu- sion.Whenthecoldcureresinwaspolymerised, the denturewasremovedfromthemouthandfinished. Since the metal housings enable highly elastic anchorage via rubber O-rings, the denture is still supportedbythesofttissueandtheloadontheim- plantsisreduced.Thus,immediateloadingispossi- bleiftherequiredprimarystabilityisobtained, asis usually the case in the mandible. The resilient con- nection concept without contact of metal to metal (soft loading) should also reduce the risk of over- load of the implant or the surrounding bone bed as observedintheFEManalysisdescribedabove.How- ever, a scientific verification of the interface re- mains to be done. _Conclusion As shown by both patient cases, mini dental im- plantsareausefulalternativeorcomplementtoim- plants with a conventional diameter. The patient is often spared complex augmentative measures which are time-consuming and invasive. In addi- tion, new treatment options are created for med- ically compromised as well as anxious patients. Af- terathoroughevaluationoftherisksandbenefitsof a treatment involving mini implants and provided that they are placed by an implantologically expe- rienced dentist who follows the insertion protocol, excellent clinical results can be obtained._ Editorial note: A complete list of references is available fromthepublisher.
